Online registration is open!! You will need to log in through www.teacherease.com to complete registration. It will then prompt you through the necessary steps. All unpaid fees from last year and new fees will be applied in order to complete registration.

In person registration is Wednesday, August 6th from 9am-1pm and Thursday, August 7th from 1pm - 6pm. In person registration is for new students, but returning students' families may turn in any physicals, shot records, and doctor exams at this time. If you do not have access to a computer, please come to in person registration and we will have a computer for you to complete registration on.
If your student is entering any of the following grades, below is what the state requires for students to attend public schools.
ENTERING PRESCHOOL:
Official Government issued Birth Certificate
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history.
ENTERING KINDERGARTEN:
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history section must be completed and signed by parent.
Eye Examination must be performed by a licensed optometrist within one year prior to the first day of school and submitted before October 15 of the school year. Parent signature is required on IL form.
Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be on IL form & completed within 18 months prior to the May 15 deadline.
ENTERING 2nd GRADE:
Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be on IL form & completed within 18 months prior to the May 15 deadline.
ENTERING 5th GRADE:
Sports Physical if planning on participating in sports and compliant with all immunization requirements.
ENTERING 6th GRADE:
Physical Exam (Sports Physical not acceptable, separate physical needed) * Physical Examination must be completed within one calendar year prior to the date of entry.
Complete Immunization Record signed and stamped by a doctor, including (1) Tdap, (2) Varicella (1) Meningitis (or Certificate of Religious Exemption if applicable).
Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be on IL form & completed within 18 months prior to the May 15 deadline.
ENTERING 7th & 8th GRADES:
Sports Physical if planning on participating in sports and compliant with all immunization requirements.
